Referring a Student For Services

When a student appears to be displaying academic difficulties which are not congruent with the perceived ability of the student, you may question whether there is a possible disability present. However you should not make any statements to the student which could be perceived to be either a diagnosis or a presumption of fact.

Instead, we suggest letting the student know that there are a number of resources available to them at Mercy College including tutoring services through the Center for Academic Excellence and Innovation, counseling services through the Counseling Center, and accommodations through ACCESS. Be sure to give the student our contact information.

If a Student Discloses A Disability

Provide the student with the contact information for The Office of ACCESSibility.

  • Include the student’s name and CWID number
  • Provide an overview/ summary of interaction/ disclosure
  • Let the student know someone from The Office of ACCESSibility will be reaching out to them YOU SHOULD DO BOTH
Friendly Reminders
  • DO NOT ask a student what their disability is. You only have to know what their accommodations are. The student can choose whether or not to reveal that confidential information.
  • DO NOT ask a student about information regarding accommodation issues when not in a private setting. For example do not ask student how they are doing with the note taker in a public setting.
  • DO NOT reveal any information to any parents or acquaintances of a student with a disability if they have not completed a FERPA.
  • DO NOT promise any accommodation to a student unless it has been approved by the Office of Disability Services.
  • DO NOT save a copy of disability related information in a student’s file or scan into Xtender. This information is confidential and must be stored in Disability Services.
  • DO feel free to ask students about how you can better meet their needs and address their concerns.
  • DO let students know that they can receive reasonable accommodations to remove any barriers that their disability may present.  “Reasonable” varies in every situation. Accommodations are given on a case-by-case basis so avoid giving blanket statements about accommodations.

Syllabus Statement

As per the Provost's Office, it is required that all syllabi contain the the following statement for students with disabilities. Additionally, please be sure to make this announcement to the class on the first day. In this way, faculty can help facilitate success for students by increasing awareness of a vital campus resource

Forms to Expect from Students

Accommodation Memorandum

  • Any student who has registered with ACCESS will present you with an Accommodation Memo like this: Accommodation Memorandum Sample.pdf
  • The memo is valid only for the semester listed
  • To receive accommodations, a student must present you with this form. The memo lists the student's approved accommodations. This memo lets you know what accommodations you legally have to provide the student.
  • When presented with the memo it is typically a good time to have or set up a private meeting to discuss their accommodations and what impact it will have on course work and exams.
  • If you need additional information about the accommodations listed, please contact ACCESS at
  • Any additional concerns regarding an accommodation listed on the Accommodation Memo, please reach out to or (914)674-7764

Proctor Request Form

  • For students who are approved to receive extra time on exams in a separate location, the student will submit this form to you at least one week before the scheduled exam.
  • Please complete it fully. ACCESS cannot schedule a proctor without all of the necessary information.
  • It is the student's responsibility to provide this form to you and return the completed form to the appropriate campus testing office

  • Student will need this form filled out completely to submit their online request for a proctored exam
